It's obvious he has doubts and questions his leadership skills and not to mention doesn't want to get his friends injured, captured or killed. But being a leader means overcoming those fears and crafting plans that would see them through their ongoing problems.
Thomas is the only character I can see surviving to the end of The Death Cure unless the movie (or the book) throws a curveball and he's surprisingly killed off. I guess I have to wait and see.
